Enter landlord information
Provide the legal name of every landlord listed on the lease, along with a current mailing address and phone number. If a representative (paralegal, property manager) is signing, capture their LSUC number and firm details too.

Add tenant and property details
List every adult tenant on the lease — exactly as their name appears on the lease. Then enter the rental unit address including unit number, city, province, and postal code.

Itemize the rent owing
For each rent period that has unpaid amounts, enter the period start and end dates, the rent charged, and the amount paid. The N4 requires a row for each separate period — not a lump sum.

Choose service method and notice date
Pick how you will serve the notice (hand, courier, or mail). The termination date is then auto-calculated by adding the minimum notice period (14 or 7 days — 7 for monthly and yearly N4s served on or after September 21, 2026 under Bill 60) plus any service-method adjustment.

Sign the form
Type your name, draw a signature, or have a representative sign. The signature date must match or follow the notice date.
